Eldershield - upgrade or not

Hi dear all
As you know once we have turn to 40, we are auto cover with baisc eldershield ( 3 out of 6 ADL for 6 years) unless we have opt out the plan.
I cant make up my mind should I upgrade basic plan to Eldershield Comrehensive Plan (which is 2 out of 6 ADL for lifetime). I am afraid of premium when getting older, no job no income although pay by CPF medisave , CPF account can also finish if something happen.
Does anyone know we have to pay the premium until what age, is it at the flat rate ? Do you think is it necessary to upgrade? Hope someone can advise me and would like to heard your view.
