If u score 191, your PSLE result slip will indicate that you qualify for Express/Normal (Academic) streams.

You can pick either one. Up to you.

If you do not indicate which stream, MOE will automatically assign you to the more demanding one, ie. Express stream for this case.

It falls under this stream : 188 - 199 Express / Normal Academic.
This means you should choose either Express or Normal Academic stream.

Got a question on this move towards SBB. Even with subject based banding, to do O levels at Sec 4, one has to be in Express right? Can a NA student who somehow was not given the chance to switch to Express, but taking subjects at higher levels through SBB, be still allowed to take O levels at Sec 4 - or needs to take at Sec 5?

NA students in SBB can take some O Level subjects at Sec 4 with the rest of the Express students.
It depends on which batch your child is in...MOE is going to slowly phase out the N level exams for NA at Sec 4. Moving to a “common exam” for Express and Normal at Sec 4. No more Sec 5. Possibly use Sec 4 results to apply for PFP.
