The TAS vocab book might be such a book. I saw a teacher carrying some loose sheets of the vocab book and asked to look at it, they will group similar words together and some of the words are synonyms and explain the difference between them which I feel is very good as most of the time the vocab options are so close. I did not look at whole book but from what I saw, it seems to cover a number of similar words.

For p6, you can find that there are some sections under the Creative Vocabulary under Redspot Pubilcation that has questions on Synonyms/Antonyms. Maybe there are some other books under Vocabulary section. As for Secondary, I'm not sure because my kids still in primary schools.
